APPLICATION FOR FINANCIAL AID - INSTRUCTIONS
READ ALL INSTRUCTIONS CAREFULLY BEFORE
FILLING OUT THE APPLICATION.
Enclosed is the Financial Aid application for the 2003-04 school
year. To avoid errors and delays in processing, when you fill out
the form be sure to complete all items; do not leave any boxes blank.
Enter only one amount in each box. Write only in the response boxes
or answer spaces. Do not write in the margins of the form or outside
the answer boxes or spaces. Enter a zero (0) if no entry applies.
Where you are requested to attach an explanation please do so.
The explanation may be written on page four or you can attach additional
sheets. Failure to provide an explanation may be cause for rejection
of your request for financial aid.
Please type or print all responses carefully in ink. Do not use
symbols such as >, <, ~, or N/A. Do not use words such as
"unknown," "same," or "none." Round
off all figures to the nearest dollar. Do not use cents. For 50
cents or more, round up; for 49 cents or less, round down. Enter
amounts in United States currency. Any amounts entered for 2003
that are $5,000 or more than those given for 2002, attach an explanation.
The application must be signed (page1). For married couples, both
parents must sign the form. Applications without the required signatures
will be returned. Be sure to keep a copy of the application. Copies
will not be supplied once received by HFS.
Acceptable unusual expenses (item 9, page 3) include nursing home
care, legal fees, closing costs for home purchase or refinancing,
sewer, street and water assessments (installation only), unreimbursed
tuition for parents' education, and uninsured natural disasters
(flood, fire, storm damage, etc.). Do not include payments for appliances,
car payments and payments for indebtedness, charge account or installment
purchase payments, charity or church contributions, child care payments,
school/college expenses for children, payments for commuting, household
help expenses, payment for routine house repairs, payments on insurance
premiums, payments for condo/association fees, mortgage payments,
payments for retirement plans. Be sure to attach a detailed explanation.
For item 15 on page 3, outstanding personal indebtedness would
include educational indebtedness for parents (past); encumbrances
(lien, personal loan for down payment, etc.) against home or other
real estate; general medical and dental expenses; indebtedness for
purchasing investments listed in item 14; living expenses if business
failure, prolonged illness, unemployment, etc.; past business debts;
personal debt to purchase business; natural disasters not covered
by insurance (flood, fire, storms, etc.); funeral expenses (past);
and legal fees (past). Do not list items already in items 8 &
9. You may be required to verify your indebtedness. Be sure to attach
a detailed explanation.
